PAISLEY
January 8, 196972, Wednesday, James Lee Paisley, of Virden.
Mr. James Lee Paisley, age 34, married, three dependent children, came to his death at or about 3:45 p. m. January 8, 1969, from mutilating internal injuries accidently received when he was caught between the ground and journal box on a railroad coal car which had been struck by 11 motorless run-away railroad coal cars near the tipple while preforming his duties as a laborer at the Freeman Coal Company Crown mine, Farmersville, Montgomery County, Illinois, where he had been employed for 10 years.
Mr. Paisley was blocking a loaded railroad car coupled with six empties. Theses cars were located under the tipple on track #4. His job was to block the loaded car and relocate the wire rope of the car retarder. The car retarder manouvers the railroad cars while they are being loaded with coal under the tipple. As he was preforming his duties, he failed to see the five run-away empties coming down the track four from the empty yard which is located approximately 1900 feet from the tipple. The impact of the run-away cars colliding with the blocked cars dislodged the blocking and broke the hook on the wire rope of the car retarder. This allowed the cars to roll forward, apparently catching Mr. Paisley between the surface of the ground and the journal box of the railroad car. Suffering from multiple injuries to his left side, an ambulance was immediately summoned and he was taken to St. John's Hospital in Springfield. The accident occurred at approximately 2:30 p. m. and he was admitted to the emergency ward at 3:25 p. m. he died 20 minutes later at 3:45 p. m.       Mr. Alfred Lowe and Mr Leo Edward Drabant were the employees in charge of riding the empty railroad cars down the hill to the tipple. There had been approximately 50 cars brought down that shift without any trouble prior to the five run-aways. They stated, however, that the change in the weather had allowed moisture to accumulate on the rails making them slick. When they applied the brakes to slow the cars, the wheels started sliding, and both men jumped from the cars. Once the cars started sliding, it is hard to regain control of them. This is what allowed the collision of the railroad cars.

ROSS
January 9, 196871, Tuesday, Harry J. Ross, of Carlinvile
Mr. Harry J. Ross, age 45, married, four dependent children, came to his death on January 9, 1968 at 4:10 a.m. while performing his duties as a top labor/shift leader and high lift operator at Florida Coal Caomapny, Little Dog Mine located at Gillespie, Macoupin County, Illinois where he had been employed for about four years.
Mr. Harry Ross, being the shift leader on top, had talked with Mr. Henry Dubbs, hoisting engineer, and Mr. Joe Cheragotti, shift leader, at the beginning of the shift in regard to material that was to be sent below on that shift. Mr. Cheragotti had requested that four material car loads of roof bolt blocks and one car of expansion shells be sent below. For this to be done, five empty cars were to be sent out on top from underground at a prearranged time of 2:00 a.m. to provide enough time for loading of material. The five cars were sent out on top as scheduled, and shortly thereafter repair work on the bonnet of the south cage was started and then completed at approximately 3:45 a.m.
During the intervening time between the beginning of the shift and the time the five material cars came on top, Mr. Ross informed the hoisting engineer that he was going to send only four cars of material below so that the extra time could be spent cleaning on the tracks in the area of the preparation plant. the lowering of the loaded material cars, sent below on the north cage only, commenced shortly after completion of the repair work on the south cage. Assuming the cage was "free" after the fourth car was taken off below because of the change of plans of Mr. Ross, the hoisting engineer parked the north cage above the ground landing in the normal "free" park postition.
Mr. Ross with the assistance of Mr. hiram Turner, top laborer, prepared to push the fifth car of material loaded with expansion shells onto the cage not realizing that the cage was above the ground landing. It should be noted that on this morning with the temperature of the air in the low twenties and because the hoist shaft is the return air shaft, that visibility at the shaft was, at times, obscured by dense steam although overhead light s are provided and were on. As the loaded car tipped forward into the shaft, it pulled Mr. Ross and Mr. Turner forward. Mr. Ross was pulled into the shaft by the falling car and fell 368 feet to his death. Mr. Turner who was on the left side of the car caught hold of the signal bell pipe and was able to regain balance. Mr. Ross came to his accidental death by skull fracture and multiple injuries.
